MAIN GOAL:

SPECIFIC GOALS:

  1.   5. Promote continuity of care for patients with cardiovascular disease through programs with Primary Care as an essential part of patient care.

  6. Establish multidisciplinary collaborations (Nursing, Endocrinology and Nutrition, Physical Medicine and Rehabilitation, Social Work, Internal Medicine, etc.) to guarantee the quality of patient care in each of the processes.

  7. Promote research by carrying out multidisciplinary and multicenter studies on an annual or biannual basis.

  8. Promote cardiovascular health at the population level through prevention and health education strategies, encouraging healthy lifestyle habits and ensuring the safe and effective use of medication. This includes collaboration with other healthcare professionals, the design of health promotion programs, and participation in institutional and community initiatives to reduce the incidence of cardiovascular disease.
